Critics say the 65 year old prime minister, Shinzo Abe, has failed to take the lead in dealing with the epidemic, leaving the task largely to his health minister.
Now, doubts about Mr Abe's leadership are likely to erode declining public support, with a weekend newspaper survey showing that dissatisfaction with Mr Abe's government has exceeded support for the first time since July 2018.
In turn, that could derail the bright future: the successful Tokyo Olympics, scheduled to open in July, with Abe leading his party to victory in the election, and perhaps even a rare fourth term at the end of his term as leader of the ruling party in September 2021.
"Where is the leader?" Asked Gerry Curtis, an expert on Japanese politics.
"Even now, he's invisible, he's not shouting to the public, he's not mobilizing," added Curtis, an emeritus professor at Columbia University. "I think he will suffer if this situation continues."
Since Abe became prime minister again in December 2012, he has been through several crises.
